HPFT - Social Care in the Community
Hertfordshire Partnership University NHS Foundation Trust (HPFT), an NHS mental health and community care provider, is developing an open framework for community-based support and homecare services for adults with mental health needs across Hertfordshire. Appointed providers will deliver support tailored to assessed individual needs, preferences and intended outcomes, responding to referrals that specify required arrangements, timescales and other service-user requirements. Providers must maintain capacity, propose suitable approaches and commencement dates, complete the standard Provider Response to Referral or equivalent secure electronic submission, and support service-user choice in accordance with the Care Act 2014. The framework is anticipated to operate for up to eight years, beginning with a two-year period that HPFT may extend by one year and reopen at two-year intervals; framework rates will apply to call-off contracts. No estimated contract value or specific delivery sites are stated. The critical qualification requirement is the ability to meet the full assessed referral requirements for adults with mental health needs within the applicable geographical lot and service type.
